Assessments & Referrals

Our home manager will arrange a person centred assessment and complete transition plan to move into our homes. To make a referral please call us on 03337729464 or email info@prosperityhousinggroup.co.uk  

We will arrange an assessment within 48 hours of receiving a referral. 

Referrals can be made by a social worker, care manager or the individual’s carers. The individual will have a care manager who acts on their behalf and must be consulted before an assessment can take place. The assessment looks at the whole person to determine if our homes are suitable for them and they will be able to achieve the most positive outcomes. We assess their personality, skills, abilities, what’s important to them, what are their goals, where they want to live and their overall needs to support them to become independent,  happy and live fulfilling lives. 

Transition Plan

We encourage the care manager and family to visit our homes once the assessment has been completed so they can meet our care team and see our service for themselves. When the placement is agreed by the care manager, individual and family. The care manager will then apply for the funding, the start date is agreed and a transition plan is created, to ensure that the transition is as smooth as possible. Each transition plan is based on individual need and choice of the individual.  

Each individual will be allocated a key worker who is responsible for their wellbeing. The key worker will take time to get to know the individuals personality, and care needs. They will also be supporting the transition to ensure that it is successful.

We meet with Commissioners and the Local Authorities on a regular basis to work together to develop specialist services for the future and maintain a great relationship.
