Sarah
Sarah

Sarah joined our service at 21 years old with a diagnosis of Autism and OCD. She had lived with her parent’s her whole life and moved into our 3-bedroom bungalow unable to complete most of the daily living tasks or access the community independently. Our service provided person-centered support to assist and guide Sarah to develop her independence and confidence. At 22, Sarah now manages her own finances, administers and manages her own medication and is able to travel using public transport independently to visit her friends and take part in activities in the community. Sarah is able to socialize with other service users to take part in activities without getting anxious and is able to understand and follow her safety plan when she is in the community independently.

Sarah is now able to enjoy her life as an independent and confident woman.

Daniel
Daniel

Daniel moved into Paulas House in 2023 with a diagnosis of Schizophrenia and joined us after a stay in hospital due to his mental health declining. When he joined us, he needed support with daily living tasks, lacked motivation and confidence and required reminders to attend appointments to manage his mental health. Our staff worked with Daniel providing person-centered support to target areas that he needed help and guidance in and grow his confidence with other aspects of his life. After 3 years working closely with our staff, Daniel is now able to attend the gym consistently to improve his physical and mental health and attend college to complete a construction course to open up new job opportunities.

Daniel has now built up his independence to where he can manage his own cooking, cleaning and laundry and attend his appointments consistently and independently to support his mental health.

Sophie
Sophie

Sophie moved into our female service when she was 24 years old and had a diagnosis of Schizophrenia, Depression and Anxiety and Autism. She had lived with her parents previously and spent a lot of her adult life in hospital due to her mental health. When she moved in, she needed support with budgeting, motivation, meal planning and confidence to participate in activities. Our service provided person-centered support to assist and guide Sophie to develop her independence and confidence. Now Sophie is 26 and she engages in support from staff with a budgeting plan to help her save for her future, has joined her local slimming world group and gym and has just passed her college course in Health and Social Care with top grades and is choosing which university to attend.

Sophie is now able to enjoy life, access emotional and financial support from our staff and improve her physical health.

Neelam
Neelam

Neelam moved into Success House 2022; she had a diagnosis of Schizophrenia and had lived with her parents for most of her life. When Neelam joined us, she could not complete her daily living tasks without support or prompting and she needed support with budgeting, motivation and confidence as she could not go out into the community without the support of staff. Our staff worked closely with her providing person-centered support plans to help and guide her to develop her cooking skills, money management, cleaning and laundry skills, independence and confidence. In 2026,

Neelam is able to manage her money with a budgeting plan, she attends the local library and community center to make friends and participate in activities.

Neelam has built up her confidence to where she is able to travel in the community independently without the support of staff, manage her own cleaning and laundry and attend her appointments consistently to manage her mental health.
