Personalisation
No two people experience mental illness in the same way. No two recoveries look identical. At PHG, we build every single care plan from scratch, informed by clinical history, personal goals, cultural background, living preferences, and the individual's own definition of what recovery means to them. We work alongside each client, their family (where appropriate), their care coordinator, and their commissioning team to create a genuinely personalised support package that evolves as they progress. This is not a tick-box exercise. It is a living, breathing, regularly reviewed plan that puts the person, not the system at the centre.
Prevention
The cost of mental health crisis, to individuals, to families, to the NHS, and to society, is enormous. PHG's entire model is built around getting upstream of crisis before it happens. We achieve this through: Proactive risk monitoring: our clinical team identifies early warning signs and acts before deterioration becomes crisis Crisis prevention planning: every resident has a bespoke crisis plan embedded in their care package from day one Medication management: we ensure safe, consistent, and effective use of prescribed medications to maintain stability Daily structured support: routine, activity, and human connection are the most powerful crisis prevention tools there are Early intervention: when things shift, we respond immediately, in-house, before external services need to be involved
Rehabilitation
Activities of daily living: cooking, cleaning, personal care, budgeting, shopping Medication self-management: supporting residents to take increasing ownership of their own health Social skills and community integration: reconnecting with the world outside Education, employment, and vocational development: building futures, not just managing presents Emotional regulation and resilience: the psychological tools to navigate life's challenges without crisis
We are fundamentally a prevention service. Every element of our care model is designed to prevent crisis escalation, prevent readmission to inpatient care, and prevent the chronic institutionalisation that costs individuals their freedom and the NHS millions of pounds every year. This is not a tick-box exercise. It is a living, breathing, regularly reviewed plan that puts the person — not the system — at the centre.
