Specialist Forensic Mental Health Supported Living

At Prosperity Housing Group, we understand that individuals with a forensic history face a unique and complex set of challenges when transitioning from secure settings into the community. Our Specialist Forensic Mental Health Supported Living service is designed to bridge that gap, providing safe, structured, and genuinely recovery-focused placements that protect both the individual and the wider community.

We work in close, ongoing partnership with Multi-Agency Public Protection Arrangements (MAPPA), probation services, forensic community mental health teams, and other key statutory partners to ensure every placement is carefully planned, consistently monitored, and responsive to changing needs.

Who we support

Our forensic supported living service is designed for adults who:

Have a forensic history and are transitioning from secure psychiatric units, prisons, or high-support environments

Are subject to MAPPA arrangements or probation supervision

Have co-occurring mental health needs, personality disorders, or neurodevelopmental conditions

Require a structured, supported environment to safely reintegrate into community life

Have previously experienced placement breakdown in less specialist settings

A Service Built Specifically to Meet the Demands of Section 117

We are not a generic supported living provider that happens to accept Section 117 referrals. We are a specialist mental health supported living provider whose entire model is designed to deliver the outcomes that Section 117 aftercare requires. Every Section 117 aftercare placement begins with a thorough, structured needs assessment conducted by our Registered Manager in collaboration with the referring social worker the CMHT, the ICB, and the local authority. This assessment identifies:

-The specific mental health needs arising from the person's mental disorder

-The risk factors that need to be managed in the community

-The practical and social support needs that must be met to prevent deterioration

-The individual's own goals, preferences, and recovery aspirations

-Any forensic or safeguarding considerations that require specialist planning

This assessment forms the foundation of the person’s individual care plan, a legally sound, robust, person-centred document that clearly articulates the aftercare services being provided and why.

What Our Service Provides

01. 24/7 Specialist Support

Our trained forensic support workers are available around the clock. Every member of our team receives specialist training in forensic mental health, risk management, trauma-informed care, and de-escalation, ensuring consistent, confident, and compassionate support at every hour of the day.

02. Individually Tailored Risk Management

We do not apply a one-size-fits-all approach. Each individual has a bespoke risk management and support plan, developed in direct collaboration with their forensic care team, probation officer, and MAPPA coordinators. Plans are reviewed regularly and updated in response to any changes in presentation, circumstances, or risk profile.

03. Structured, Therapeutic Environments

Our properties are carefully selected and adapted to provide the right balance of structure and independence. Clear routines, boundaries, and expectations are established from the outset, not as restrictions, but as frameworks that support stability, predictability, and long-term progress.

04. Multi-Agency Coordination

Effective forensic-supported living depends on genuine collaboration. We attend all relevant multi-agency meetings, contribute meaningfully to care and risk planning, and maintain transparent, timely communication with all partners involved in an individual's care and supervision. We understand our role in the wider system and take it seriously.

05. Pathway Planning and Step-Down Support

Our service is designed with progression in mind. We work alongside individuals and their care teams from day one to identify goals, build skills, and create realistic pathways toward greater independence, whether that means moving to lower-support accommodation, employment, or sustained community living.

We believe that a forensic history does not define a person’s future. Recovery is possible, reintegration is achievable, and with the right support in place, individuals with the most complex histories can and do go on to live meaningful, contributing lives.

every person we support is treated as an individual, with their own history, strengths, and aspirations. We do not reduce people to their index offence.

Recovery-focused practice
