What Are Outcome Measures?

As a CQC registered Supported Living Service in The West Midlands, our aim is for our service users to have a better quality of life. It’s more than just improved levels of ability, health, employment or housing status.

Our values are to engage the people that chose to be supported by us to feel safer, be healthier, and have an improved quality of life to prevent relapse.

Our Prosperity Pathway Framework measures the effectiveness and safety of the service and the interventions as well as take into account the personal goals of the resident outlined in the care plans. ⁣The outcomes measure the change and progress towards personalised, therapeutic goals.

This combination is flexible and meets the needs of the individual receiving care while enabling the quality of service/treatment to be monitored.⁣Our approach to care is person centred as it allows residents to measure their own progress with the support of their key workers and staff.
